Criteria for admission to surgery :

Patient must be at least eighteen years of age. Patients under twenty years old must obtain consent from a parent or legal guardian.   Patients must be diagnosed with at least one of the following disorders: gender identity disorder, gender dysphoria, or associated conditions (anxiety, depression ...etc).   Patients must have a one year history of living full-time in the cross-gender role.   Physical on hormones, have had treatment with antiandrogens or/and hormones for at least 1 year.

Educate yourself by doing the following :

Consult a Psychologist and Endocrinologist to learn how to get physically and mentally ready for SRS.   Find answers to your questions and concerns about SRS before making a decision. Get Ready for the Surgery

1. Physical preparation   Discontinue Hormone Treatment     Patients must discontinue hormone treatment at least fourteen days prior to surgery. Hormones should be halted to reduce the risk of thrombosis (blood clots). Oral tablets shoud be halted two weeks prior to surgery and injectables should be halted four weeks prior to surgery. Oral antiandrogens can be halted three days before surgery (four weeks if injectables). Aspirin and smoking should be halted two weeks before surgery.   Medical check up     Patients must complete a health check up within three months prior to SRS. Patients must also be confirmed by a private physician to be free from serious medical deseases and must pass the following blood tests: - CBC, HIV Electrolytes, FBS, Creatinine Urinalysis - Alkaline Phosphatase, Chest X-ray - SGOT LDH EKG   2. Document preparation   Two letters are required from foreign patients. One letter must be from either a medical doctor or a psychologist that states the person is a candidate for SRS. The other letter may be from any doctors, showing proof of having been on hormones for at least one year   Health history   3. Submit an Application Form   BIO data   Scheduling operation   Health history   Photo jpg' format. of the face should include frontal, exact profile, and lateral views - and be close-up, detailed and esp. show the scalp hairline, brow bossing area, jawline, and throat. There should be no facial expressions (e.g. smiling, pursing of the lips, etc).   4. Confirm the following :   Surgical date   Clinical consult date   Hospital admission date   Get invitation letter from the surgeon   5. Passport and Visa   US citizens can call San Francisco information (415) 744-4444 or you can search under "U.S. Government offices" in the white pages to find the phone number of your local passport office.   For most tourists, a stay of no more than 30 days does not require a VISA. For further information on Thailand's entry/exit requirements, please contact the Thai consulate in your country or your travel agent.   Citizens who wish to renew a passport under different gender should contact a clerk at the passport office and provide the following information :     - Female driver's license     - The court decree of name change     - Photographs     - Letter from doctor giving the date of surgery   6. Travel Arrangements   Hotel and Transfer booking. For flight arrangement , our recommended airlines are Thai International, Northwest, Lufthansa, Singapore Airline and EVA Airline which has wide seats. Make request wheelchair on return flight.   7. Packing   Pack only loose clothing. If the patient arrives alone, it is essential to travel with light airport baggage/luggage (max. 20 kgs) to avoid risking injury during the 4-6 weeks recovery period.   8. Take care of yourself, stay healthy, get ready for take off!   You should confirm your flight again, 2 days in advance, before leaving your country or if you have any changing, you have to inform your consultant immediately.
